JOHNSTON- Hazel O. (O'Leary) Zifcak, 92, died Thursday Aug. 9, 2012 at Home & Hospice Care of RI, Providence.
A resident of Johnston, she lived for many years in Burrillville. Wife of John H. Zifcak for nearly 75 years.
Born in Windsor, Nova Scotia, she was the daughter of the late Truman and Madeline (Wile) O'Leary.
